LOLOLOL mistaken years is one of my largest pet peeves about the fandom...i've noticed SO many people do it. it makes me scowl and flip back every time. right up next to that is inadequate description of physical movements--you can't have someone be holding someone else up against a wall, then be on their back, then back pressing them against the wall again without EXPLANATION. if you're going to be specific about physical placement, be CONSISTENT. (explicit description of postures/hand placements that don't in fact make anatomical sense are included with that.)
